B. Soil analysis submission forms
B1. Soil analysis form for vines and fruit
The following information is required to assist the laboratory in supplying you with a meaningful and accurate fertiliser recommendation:
- Your personal information.
- The purpose of the analysis.
- Which analysis is requested.
- Your sample marks (topsoil, subsoil 1 and subsoil 2, if relevant) and their respective depth ranges.
- Whether the samples submitted are for established crops.
- The age of the samples.
- Whether crops are being irrigated or not.
- The expected yield and growth.
This form can accommodate 8 samples with 3 different depth ranges each (24 in total).

B2. Soil analysis form for cereals and pastures
The following information is required to assist the laboratory in supplying you with a meaningful and accurate fertiliser recommendation:
- Your personal information.
- The extension ward.
- Rainfall in mm (if not irrigated).
- Your sample marks and their respective depth ranges.
- The intended crop that the fertiliser recommendation is required for.
- The crop rotation code (if applicable).
- Whether the intended crop will be irrigated or not.
- What analysis is requested.
Additional information or requests regarding your samples can be indicated under "General Remarks".
This form can accommodate 6 samples.

B3. Soil analysis form for any other plant or crop
The following information is required to assist the laboratory in supplying you with a meaningful and accurate fertiliser recommendation:
- Your personal information.
- Your sample marks and their respective depth ranges.
- The intended crop that the fertiliser recommendation is required for.
- Whether the intended crop will be irrigated or not.
- What analysis is requested.
Additional information or requests regarding your samples can be indicated under "General Remarks".
This form can accommodate six samples.
